New Patient Registration
If you live within the practice catchment area (see map opposite)you will be entitled to register with the practice. Please come to reception to complete a registration form (or download from the link below) You will need to be able to provide evidence of your current address in the form of a recent utility bill. The registration checks are carried out by our Healthcare Assistant or Practice Nurse. You will be given a health questionnaire and asked to bring a specimen of urine to the appointment. The registration check consists of a height, weight, blood pressure and urine check.You will also be asked about your past medical history. This is to give us some information about you until your medical record arrives at the practice.
Patients on repeat medication will be required to make an appointment with the GP for their first repeat prescription. Thereafter, you will be able to order repeat medication via the usual methods.
Patients who move outside the area will be asked to register with a more local GP surgery.
Each registered patient at Lindley Group Practice has a named GP who is responsible for that patients overall care at the practice. If you wish to know who is your named GP, please contact the practice. If you have a preference as to which GP that is, reasonable efforts will be made to accommodate your request. However, a named GP (whether it is one of your choice or not), will not affect your care at Lindley Group Practice in any way.
Changes to Personal Details
It is the responsibility of patients to keep us informed of any changes (name, contact number, or address) so we can update your record.
If you move outside the practice area, you are advised to register with a more local GP surgery as soon as possible so your records can be transferred.
