If you require vaccinations for foreign travel you must complete a travel questionnaire for each person (can be printed off the link below or collected from reception). Once completed, please hand into reception with your contact details. The Practice Nurse will review your medical record and contact you to arrange a suitable appointment for vaccinations and up to date travel advice.

It is important to give us 6-8 weeks notice of your intent to travel, as a second appointment may be required before you travel and it is important to be fully vaccinated. If you do not allow us sufficient notice, we may be unable to offer you an appointment and you will be signposted to other local travel clinics.
