How To Register

If you live within the practice catchment area, you will be entitled to register with the practice. Please come to reception to complete a registration form (or download from the link below) You will need to be able to provide evidence of your current address in the form of a recent utility bill. The registration checks are carried out by our Healthcare Assistant or Practice Nurse. You will be given a health questionnaire and asked to bring a specimen of urine to the appointment. The registration check consists of a height, weight, blood pressure and urine check. You will also be asked about your past medical history. This is to give us some information about you until your medical record arrives at the practice.

Patients on repeat medication will be required to make an appointment with the GP for their first repeat prescription. Thereafter, you will be able to order repeat medication via the usual methods.

Patients who move outside the area will be asked to register with a more local GP surgery.
